Granite School District provides transition support, both academically and behaviorally to all incoming 6th, 7th, and 9th grade students at the Jr. Highs and High Schools by implementing a transition success course. The goal of these courses is for students to successfully transition into Jr. High and High School and to advance with all required graduation credits.
Transition Course Organizational Structure
Ongoing Support:
Academy PLC
- PLC Team includes:
- Transition Course Teachers
- Core Course Teachers
- Counselor(s)
- Administrator(s)
- Any other teacher connected to transition grade level
- Meets monthly
- reviews data
- Creates Tier 1 and Tier 2 interventions
Transition Course Class
- All students registered in class for 1st semester
- 2nd semester course for students needing additional support and/or interventions
- Use of district-wide standards, objectives, benchmarks, and curriculum map
- Use of Grade Level Ambassadors/Peer Mentors
Individual Student Focus
- Weekly conferences with mentor teacher
- Navigating School/Transition supports
- High school and career planning
- Option to complete online coursework
- Interventions as needed
