Student Travel
The Granite School District Property and Liability insurance coverage is provided by the State of Utah Division of Risk Management, which will prepare a Certificate of Insurance when students travel to school programs or to perform on location. Owners of facilities require proof that our schools and students are covered by insurance. To request a Certificate of Insurance for your student event, please contact Darci Williams via email or call 385-646-4409.
Auto Accident
State Risk Management also provides liability, collision and comprehensive insurance coverage for Granite School District vehicles. The district provides secondary liability insurance coverage for employees who drive their own vehicles on district business.
Injury
If a student is injured at school, a school employee must complete a student injury report.
- This injury report is online through RedCap. The Utah Department of Health designed this document. To use this online report, you must have a password. Please contact Vanonda Kern or 385-267-6528 to obtain a password and access to student injury reporting.
If an adult, who is not an employee or volunteer, is injured while on Granite School District property, an Adult Injury Form (PDF) should be completed. Fill out what is applicable, add current address of the injured adult then send in district mail to our Risk Management office.
If an employee or volunteer is injured while on Granite School District property, all injury reports should be processed as Workers Comp injuries. All documentation and questions about Workers Comp should be sent to Brenda Byrnes, in Human Resources.
If a student is injured at school or on other district property, State Risk Management provides liability coverage if the district or its employee is negligent or liable. (The state will determine liability.) Granite School District personnel should never accept liability or tell others that the district is liable. This must be done only by a State Risk Management employee.
